Cat health & vaccination record
A single place to find what happened, when it happened and what your vet asked you to do next. Keep original clinic documents alongside this summary.
One page · Free · No sign-up. Blank PDF for handwriting or annotation on your device; not an interactive form.

Start with the record you already have
Gather your clinic’s vaccination record, recent visit notes and any current medication instructions. Add the cat’s name, identifying details, microchip number if known and clinic contact at the top of the sheet. Copy dates carefully. If something is uncertain, write “not confirmed” and ask the clinic rather than guessing.
Record the event and the source
Use a row for each vaccination, visit or other care event. Write the date, the exact name on the clinic record and the clinic or source document. The notes column can hold the reason for a visit or where you saved the full report. Keep invoices, certificates and results: this personal summary is not an official vaccination certificate or travel document.
Use your vet’s follow-up dates
Fill the next-date column only from a date agreed with your vet or written on the care record. There is no pre-filled vaccine schedule on this printable because the sheet organizes your information. If a date has changed, cross out the earlier one clearly and record the new instruction. Keep medication instructions with their original label.
Bring a short, current summary
Before an appointment, review the last few entries and note the questions you want to ask. Bring the relevant originals as well as your summary. After the visit, add the new record while the details are fresh. When the sheet fills up, number the pages and keep them in date order; do not overwrite earlier care history.
Questions about the printable
Is this a vaccination certificate?
No. It is a personal record sheet. Ask your clinic for official certificates, signed records or travel paperwork.
Does it tell me which vaccines are due?
No. Record the vaccines and next dates your vet recommends for your cat. Blank spaces are not a recommended care schedule.
