Cat daily observation log
Write down what you noticed while it is fresh. A seven-day sheet keeps litter box, food, water and other observations together for a conversation with your vet.
One page · Free · No sign-up. Blank PDF for handwriting or annotation on your device; not an interactive form.

Give the notes some context
Write the cat’s name and the week at the top. Use a separate sheet for each cat where you can identify whose activity you observed. In a shared home, write “unknown cat” or “not observed” when you cannot tell. A blank cell should not silently mean that everything was usual.
Record what happened, in plain language
For appetite, note the food offered and what was actually eaten if you know. For water, describe the change you noticed without inventing a measured amount. Record litter box observations and the time of any vomiting or hairball event. The notes space can hold a change in routine or a question for the clinic. Be specific: “left half of breakfast at 8 am” is easier to discuss than “off food”.
Keep observations distinct from conclusions
This sheet has no normal ranges, symptom score or diagnosis. It cannot tell you whether a change is harmless. If you are concerned about your cat, contact your vet rather than waiting to finish the week. Your vet can advise which observations and measurements are useful for the individual cat.
Make the record easy to share
Keep the sheet somewhere you will remember to use it, or add notes on your device. Before sharing, check that each entry has a date and that it belongs to the right cat. Include the relevant time period and your main question. Keep a copy for yourself. The downloadable PDF has no form submission and sends no cat records to this website.
Questions about the printable
Is this a diagnosis or symptom checker?
No. It is a blank record of observations. It does not recommend treatment or decide when care is needed.
Can I record several events in one day?
Yes. Add times in the cell or use another copy for extra detail. Do not combine several events into a single count if the timing matters for your discussion.
